If you need your child’s medical record transferred to a non-CPCMG physician, you will need to submit a medical release form. HOW TO ACCESS YOUR CHILD'S MEDICAL AND/OR IMMUNIZATION RECORDS: We can then decide if we need to request copies of prior medical records. Therefore, please inform our office if your child has been seen by another physician. It is important that your child’s medical records be complete. This is a continuous record that includes visit notes, lab results, x-ray reports, diagnoses and treatments, and any medication given. The medical record is the physician’s documentation of the care given to your child.
